    By submitting this application, I affirm that the facts set forth in it are true and complete.  I understand that if I am accepted as a volunteer, any false statements, omissions,or other misrepresentations made by me on this application may result in my immediate dismissal.

    OUR POLICY
    It is the policy of this organization to provide equal opportunities without regard to race,color, religion, national origin, gender, sexual preference, age, or disability. If you are under 18 years of age, a parent or legal guardian must also sign a waiver on your behalf. If you are under 16 years of age, a parent or legal guardian must sign the waiver AND accompany you, unless you are with an organized, chaperoned group which has the Greeneville Farmers Market’s permission to participate. If you are under age 18, you must turn in a hard copy of this waiver.I hereby release the Greeneville Farmers Market, its agents, and its representatives from any liability and responsibility that may arise in connection with my volunteer duties. I also hereby consent for the Greeneville Farmers Market to use my name, likeness or program participation for public relations purposes; I understand that I will not receive compensation for any such use.If driving is involved in my volunteer duties, I hereby acknowledge that I have a current driver's license and automobile liability insurance.
     I also agree to the following requirements:
    ● To perform my volunteer duties to the best of my ability;
    ● To adhere to Greeneville Farmers Market rules and procedures, including record-keeping requirements and the confidentiality of agency and client information;
    ● To adhere to my volunteer commitments, or to provide adequate notice so that alternate arrangements can be made;
    ● To at all times conduct myself as a member of the team responsible for accomplishing Greeneville Farmers Market mission
    .My consent serves as notice that the information provided on this application is true and accurate to the best of my knowledge. I understand that any intentional false or misleading information I provide may be grounds for dismissal from the Greeneville Farmers Market volunteer program. I also consent to a voluntary background check by Greeneville Farmers Market if required to perform my volunteer duty.
